
Community Action Team (CAT)
¹ÏÉñ’s Community Action Team (CAT) is designed to function as a “hospital without walls,” with frequent therapy (at least weekly), intensive case management and support services provided by mentors and a nurse. CAT serves the entire family system to include any siblings and parents, utilizing a wraparound model of care. Our team includes case managers, therapeutic mentors, therapists and a nurse. No insurance is required. CAT services are designed for children who meet any of the following criteria:
- At risk for hospitalization or out-of-home placement
- At risk for involvement with the Department of Juvenile Justice
- At risk for involvement with or the Department of Children and Families
- Experience problems in academic settings
