Community-based care for children

When a child faces mental health issues, both the child – and their family – may benefit from community-based, in-home support to help face everyday challenges. Community-based care allows us to reach children in need wherever they are. By bringing care to those we serve, we remove barriers to treatment so children and families can get the help they need – in settings where they feel most comfortable.

Our goal is to help children, adolescents and family members succeed in their home environments, preventing the need for residential treatment, hospitalization or out-of-home placement.

Also called family-based intervention services, these in-home services are meant to help children and families needing specialized and intensive levels of support beyond outpatient counseling and psychiatric care.

Community Action Team (CAT)

¹ÏÉñ’s Community Action Team (CAT) is designed to function as a “hospital without walls,” with frequent therapy (at least weekly), intensive case management and support services provided by mentors and a nurse. CAT serves the entire family system to include any siblings and parents, utilizing a wraparound model of care. Our team includes case managers, therapeutic mentors, therapists and a nurse. No insurance is required. CAT services are designed for children who meet any of the following criteria:

  • At risk for hospitalization or out-of-home placement
  • At risk for involvement with the Department of Juvenile Justice
  • At risk for involvement with or the Department of Children and Families
  • Experience problems in academic settings

Case management for children

Case managers coordinate care and treatments for kids with behavioral health needs. Our family-centered and community-based team serves children and adolescents in Escambia and Santa Rosa counties, providing services in homes and schools, as well as outpatient office settings. Case management strives for a cohesive, holistic approach to achieve the best possible outcomes for children and families.

Family Crisis and Care Team (FCCT):

In Escambia and Santa Rosa Counties, this 90-day program provides intensive in-home crisis stabilization and care coordination for children ages 5-18 who have had two Baker Acts (or one for kids under age 10) and/or are at high risk for hospitalization, are at risk or are already involved with the Department of Juvenile Justice, or are at risk for out-of-home placement with and the Department of Children and Families. Our team includes therapists for crisis stabilization, case managers and peer specialists. With no insurance required, this program serves those in need regardless of ability to pay.

Child welfare support

¹ÏÉñ provides specialized support for families in the child welfare system, including behavioral health care for kids with specialized needs, as well as foster care group homes and intensive wraparound support for parents struggling with substance misuse.

Adolescents in Motion (AIM)

Adolescents in Motion (AIM) works to address the increase in youth overdose deaths across Northwest Florida. We provide stabilization, outreach, education and care coordination services for children and young adults ages 5-18 who have substance abuse concerns.

Our specialized team of adolescent substance abuse providers offers in-home services that focus not only on youth, but on the families, homes and communities where these youth live. We provide education, coordinate treatment needs and work to stabilize family conflicts that often accompany substance abuse. No insurance is required.
